- SAHPRA Directorate: Radiation Control uses two forms together for laser (and similar electromedical / electronic product) licensing.
- **RC011-1 (rev 1) — Details of a Transaction:** filled by the licensed supplier / licence holder to notify Radiation Control of a sale, transfer, or similar transaction. Submit to the Director: Radiation Control within 21 days of the transaction.
- **SBLM-1 — Application for a licence:** filled by the end-user (purchaser / premises owner) to legally possess and use the laser. This is the form the buyer uses after they have bought the machine.
- The seller / licence holder must give the purchaser the latest SBLM-1 at the time of sale. Sales should not send RC011 as the buyer's application form.

- A working copy of the end-user application is also seen named SBLN-1 (licence to use a non-medical laser). Quote **SBLM-1** to clients; treat SBLN-1 as the same family of end-user application, not as RC011.
- Do not prefill RC011 for a buyer who needs to licence the machine they purchased; they need SBLM-1. AM files RC011-1 as the licensed supplier after the sale.
