- Two SAHPRA Radiation Control forms are used together:
SBLM-1 (buyer / end-user)
- The purchaser (or premises owner) completes SBLM-1 to apply for a licence to possess and use the laser.
- After buying a TruCUT CO2 laser, this is the form the client uses.
- The licensed supplier must give the purchaser the latest SBLM-1 at the time of sale.
RC011-1 rev 1 (supplier / licence holder)
- The licensed equipment supplier notifies SAHPRA of the sale or transfer.
- RC011-1 (rev 1) goes to the Director: Radiation Control within 21 days of the transaction.
-
RC011 is not the buyer's application form.
